Why This Matters
HPRD (hours per resident per day) is the standard for measuring nursing home staffing, but it's often hard to understand what that ratio means for the day-to-day experience of staff and residents.
This tool converts HPRD into something more intuitive: Resident-to-Staff Ratio. That is, how many residents each nurse staff or aide is likely responsible for at a given time, based on real-world staffing patterns. That means you can better understand what those numbers actually mean for care.
Methodology
This staffing calculator uses federal CMS payroll-based journal (PBJ) data and research on staffing shift patterns to project resident-staff ratios during a given shift and day. These are rough estimates and should not be used to make legal claims, regulatory arguments, or definitive assessments of care quality.
1. Data Source and Time Period
- Day-of-week averages are based on Q3-Q4 2024 PBJ data
- Holiday periods were excluded from day-of-week averages
- For example, Thursday averages exclude July 4th and Thanksgiving
2. Staff Distribution by Shift
- PBJ data does not include shift-level detail. This calculator assumes the following distribution for RNs and LPNs: 41.0% day shift, 31.6% evening shift, 27.4% night shift.
- For nurse aides: 38.1% day shift, 34.1% evening shift, 27.8% night shift.
- These estimates are based on shift patterns observed in a study of Ohio nursing homes. Different facilities may have different shift patterns.
3. Daily Staffing Patterns
- Weekday staffing (Monday-Friday) shows higher RN and LPN coverage
- Weekend staffing (Saturday-Sunday) maintains consistent Nurse Aide coverage
- RN coverage is significantly reduced on weekends
- LPN coverage shows moderate reduction on weekends
4. Calculation Method
- Direct HPRD values are used for each position and day
- No adjustment factors are applied outside of day-of-week and shift adjustments
- Shift distribution percentages are based on study of Ohio nursing homes
